Assisted Outpatient Care Coordinator

AOT (also known as involuntary outpatient commitment and other terms) is a recognized evidence-based practice and involves petitioning local courts to order individuals to enter and remain in treatment within the community for a specified period of time. Assisted Outpatient Treatment (AOT) is not an alternative to voluntary treatment. It is a way to get services to those who refuse voluntary treatment. AOT commits local mental health systems to serve participants at the same time it commits participants to adhere to their treatment plans. Developed by patients with their healthcare providers, these plans are highly individual but typically include case management, personal therapy, medication and other tools known to promote recovery. AOT is intended to facilitate the delivery of community-based outpatient mental disorder treatment services for individuals with SMI that are under a civil court order and participants who meet AOT criteria established by state statute, receive due process protections and orders are made only after a hearing before a judge. This is a pilot project position. The AOT Care Coordinator is responsible for an assigned caseload of individuals who are under the involuntary commitment order. Provides case management functions including those of coordination of service planning, facilitation of linkages with community and Center resources, monitoring of overall service delivery and referrals of the participant to sources providing assistance with basic human needs, such as food and shelter. As coordinators of patient care, this position guides the overall plan of care by assessing needs, developing, implementing and reviewing service plans, monitoring patient progress, adjusting treatment plans as necessary and working collaboratively with the health care team and with other community resources in meeting/achieving individual service needs. Acts as a liaison and patient advocate between patient/family and service delivery personnel to ensure individuals are being effectively treated. Services are provided in accordance with accepted standards of professional practice and policies and procedures of HBH while assuring that the program operates within the guidelines established the Department of Children and Families/Broward Behavioral Health Coalition, and Broward County, in a manner that is consistent with HBH mission, vision and values. This is a 100% direct service position that could require availability to work irregular hours, including evenings. This position requires a minimum of a Bachelor’s degree in Behavioral Health Sciences. Candidate should have a minimum of (1) one year experience concentrated in the behavioral health field.
