Minimum of a Bachelor’s Degree. Minimum of (1) one-year experience with adult behavioral health population and/or substance abuse arena, or with individuals with similar human-services needs, doing assessments, discharge planning, utilization review, case management, or similar role desirable. Familiarity with: the various resources within the local community/continuum of care; the concept of individual service planning and Florida’s 65E-15 Administration Code; Medicaid billing requirements for Community Mental Health and Targeted Case Management Services. This position provides case management functions including those of coordination of service planning, facilitation of linkages with community and center resources, monitoring of overall service delivery and referrals of the person served to sources providing assistance. As coordinators of patient care, this position is responsible for an assigned caseload of individuals and guides the overall plan of care by assessing needs, developing, implementing and reviewing service plans, monitoring patient progress, adjusting treatment plans as necessary and working collaboratively with the health care team and with other community resources in meeting/achieving individual service needs. AHCA approved Mental Health Targeted Case Management Training, Certifications must be obtained within 180 days after hire. Ideal candidate should have the ability to collaborate and interact effectively with all disciplines and levels of staff; transition between working independently and collaboratively; possess a team player mentality and be able to function as a proactive member of a team by exhibiting skillful interpersonal skills. Be receptive to supervision and professional development. A valid Florida driver’s license and auto insurance coverage of $50,000/$100,000/$50,000 are required.
