A serious mental illness significantly affects a person’s emotional state and behavior to the degree that their day-to-day functioning can be impaired.

 

community-interiorHenderson’s Community Support Services help individuals by promoting recovery, hope, choice, and success. This is accomplished through effective and efficient community-based interventions which help individuals maximize their strengths to lead rewarding and productive lives.

Case Management: Works with the individual and their family to identify and prioritize needs. Together, they develop a service plan that establishes short and long-term goals and objectives for achieving them.

FACT (Florida Assertive Community Treatment): Evidence-based program which delivers “services without walls.” We recognize the therapeutic role of the home environment for providing community-based psychiatric rehabilitation, treatment and intensive case management services to persons who have not responded well to the traditional mental health model of service. FACT services are provided by multi-disciplinary teams and are available 24 hours a day, 7 days per week.

Forensic Services Team: Works closely with persons who have been deemed Incompetent to Proceed (ITP) by a court of law. Henderson’s professional staff ensure individuals receive the mental health and competency restoration services they need and want. They advocate for due process in the criminal justice system and support the individual in regaining their competency. Services are designed to assist and guide individuals in re-connecting with society and rebuilding skills in identified roles in their environment.

HILL (Helping Individuals Live Longer) Project: A primary behavioral health care integration program that provides medical care, care management and coordination, and peer support. The HILL Project follows the “Health Home” Model, which combines medical treatment with disease self management education, coordination of follow-up medical services, lab services, and peer-led wellness activities. The focus of HILL Project service delivery is achieving the “triple aim” of better care, improved outcomes, and lower costs.
