Youth Wraparound Case Manager

The Wraparound Case Manager’s main role is to assist youth and families with complex behavioral needs in identifying and working towards their desired goals and building a support system through linkage (referrals to community resources), advocacy, and coordination of services. This includes regular home visits, completing assessments and developing treatment plans, and facilitating team meetings. Plenty of training is provided. This position requires a bachelor’s degree in psychology, human services, or related field, and a minimum of one year experience working with children and families. A valid Florida driver’s license and auto liability insurance are required. Must have knowledge of mental health disorders. Ability to document all services in an electronic health record is necessary.
