This position serves as a guide who is responsible for coordinating care for clients with chronic and high risk medical and/or behavioral health conditions. This position bridges the gaps in an individual’s care. Interventions are focused on helping individuals engage in care and navigate the road to recovery. Minimum of a Bachelor’s Degree in Behavioral Sciences, Human Services or related field. Master’s Degree in psychology, social work, counseling education, mental health counseling, or related field; preferred. Valid Florida driver’s license and proof of insurance indicating minimum of $10,000/$20,000/$10,000 coverage.
