Case Manager – Florida Assertive Community Team (FACT)

This member of the multidisciplinary team has responsibility to carry out rehabilitation and support functions and assist in treatment; substance abuse services; education, support, and consultation to families; and crisis intervention under the clinical supervision of staff with professional degrees. Candidate will provide practical help and support, advocacy, and coordination, side-by-side individualized support, problem solving, direct assistance, training, and supervision to help obtain the necessities of daily living including medical and dental health care; legal and advocacy services; financial support such as entitlements or housing subsidies; supported housing; money management services; and transportation. The position requires a bachelor’s degree in behavioral science degree. Work experience with adults with severe and persistent mental illnesses or with individuals with similar human-service needs. Familiarity with the various resources within the local community. Awareness of the concept of individual service planning. A valid Florida driver’s license and auto liability insurance are required.

Case Manager

This position requires a bachelor’s degree in the Human Services field. The candidate must have a minimum of one year experience working with adults in the mental health field as well as knowledge of mental health disorders, treatment, and community resources. Excellent verbal and written communication skills are necessary. A valid Florida driver’s license and auto liability insurance are required.

Scheduler and Care Navigator

This is an entry-level, office-based position that requires at least a high school diploma or equivalent. Customer service experience is required, and we’re looking for someone with strong communication and interpersonal skills who is comfortable working in a fast-paced environment. Experience with electronic health records is a plus but not required. Ideal candidate should have basic office skills, like answering phones, sending emails, and supporting staff with administrative tasks. This role is a support position at Henderson Behavioral Health that focuses on non-clinical tasks. The Scheduling and Care Navigator’s main responsibility is to speak with potential and current clients, families, hospitals, and outside organizations to help with scheduling and maintaining client appointments. Ideal candidate must work well with both internal teams and external partners, multitask effectively, manage completing priorities, and demonstrate empathy while maintaining professionalism. Spanish speaking preferred.

Program Supervisor

Minimum of a Licensed (LMHC, LMFT, or LCSW) Master’s Degree in Mental Health Counseling, Counseling Education, Psychology, Social Work or related field. Function as the leader and a core member of a collaborative Early Treatment Program whose interventions and treatment are focused on helping individuals navigate the road to recovery from an episode of psychosis, including supporting efforts to function well at home, on the job, at school, and in the social world. This individual provides guidance, instruction, direction and leadership to the various group members for the purpose of achieving the delivery of Coordinated Specialty Care services. Assisting in designing and developing program services which include planning, budgeting, organizing, and directing all services related to the program. Work collaboratively with other professionals throughout the community and aid in data collection and reporting.  Assist in establishing performance standards and monitor compliance. Communicate and provide consultation/training to other levels of management, units, and community agencies and partners. Services are provided in accordance with accepted standards of professional practice and policies and procedures of HBH while assuring that the program operates within the guidelines established by the Navigate Model, the Department of Children and Families/BBHC in a manner that is consistent with HBH mission, vision and values. This is a position that requires a portion of time in direct service and availability to work irregular hours, including evenings, overnights, and weekends; as well as on-call duties. Knowledge of medical terminology normally acquired over one or more years of work experience in related service. Bilingual (English/Spanish) a plus but not required. Possess a valid Florida driver’s license. Exhibit proof of auto insurance coverage in the amount of $10,000/$20,000/$10,000 for liability.

Coordinator – Placement Partnership Program (PPP)

This position requires a master’s degree in a human service-related field and a minimum of one (1) year experience working with at risk children and families with child welfare involvement. Candidates with a bachelor’s degree that have significant experience with dependency, crisis support, complex needs will be considered and require ChildNet approval. The Placement Partnership Program (PPP) Coordinator responds to the urgent need to support foster care youth’s preservation of their foster care placement and re-entry into the community from psychiatric hospitals, residential treatment centers, and detention centers. The program also helps to prevent youth, who come to the attention of ChildNet, from entering the foster care system by intervening in potential lockout situations. The PPP coordinator is responsible to provide mobile, in-home therapeutic crisis intervention services to children and families, create individualized plans of care for each client, and coordinate the supports needed to sustain the identified placements. PPP coordinators work in cooperation with Dependency Case Manager and/or Child Protective Investigator, if applicable, to monitor the progress of children and families receiving services and to assess the need for further intervention. The candidate must also possess knowledge in crisis intervention, family dynamics, childhood/adolescent development, community providers and resource network, DSM5. Excellent communication skills (both verbal and written). Strong organization and time management skills. Bilingual, preferred, Spanish/Creole. A valid Florida driver’s license and auto liability insurance are required.

Youth Clinician

The Youth Wraparound Clinician provides therapy to children/adolescents and their families in an office and/or community setting including treatment planning, behavior modification, individual/family/group therapy, and outcome tracking. Candidate is required to have a master’s degree in mental health counseling, social work, marriage and family therapy, or other applicable human services degree. Candidate must have a minimum of two (2) years of experience working with children/adolescents and their families. Candidate should be knowledgeable in family systems therapy and application, behavior modification, substance abuse/co-occurring, cultural sensitivity, DSM V, and crisis intervention. Excellent communication skills both verbally and written are required, as well as the ability to work with all levels of personnel. A valid Florida driver’s license and auto liability insurance are required. The ideal candidate would be in the northern area of Brevard between Cocoa and Titusville. Position requires home visits in the evenings and weekends when necessary.

Clinician – Youth Wraparound

Provides therapy to children/adolescents and their families in an office and/or community setting including treatment planning, behavior modification, individual/family/group therapy, and outcome tracking. Candidate is required to have a master’s degree in mental health counseling, social work, marriage and family therapy, or other applicable human services degree. Licensed preferred (LMHC, LMFT, or LCSW). Candidate must have a minimum of two (2) years of experience working with children/adolescents and their families. Candidate should be knowledgeable in family systems therapy and application, behavior modification, substance abuse/co-occurring, cultural sensitivity, DSM V, and crisis intervention. Excellent communication skills both verbally and written are required, as well as the ability to work with all levels of personnel. This person will do bios, clinical impressions and therapy for Palm Beach location. A valid Florida driver’s license and auto liability insurance are required.

Peer Specialist – Involuntary Outpatient Program (IOP)

Minimum of High School degree or equivalent. Some college preferred. Paraprofessionals may have a bachelor’s degree in a field other than behavioral sciences or a minimum of a high school diploma or equivalent. Certified Peer Specialist desired; or applicant must obtain certification at the end of 12 months of employment. Must have experience in recovery from either mental illness or substance abuse by being a past recipient of public or private behavioral care services. Experience with individuals with mental illness or similar human-service’s needs, or other applicable experience desired. Previous experience in advocacy or similar roles are desirable. Experience with computer technology (using computer & PC software to support activities; Windows based programs, e-mail, especially Microsoft Office applications, keyboarding skills, printer, calculator, fax machine, MFP, copier) and other office tools efficiently in order to meet performance goals is necessary to the position. A valid Florida driver’s license and auto liability insurance are required.

The Wraparound Case Manager’s main role is to assist youth and families with complex behavioral needs in identifying and working towards their desired goals and building a support system through linkage (referrals to community resources), advocacy, and coordination of services. This includes regular home visits, completing assessments and developing treatment plans, and facilitating team meetings. Plenty of training is provided. This position requires a bachelor’s degree in psychology, human services, or related field, and a minimum of one year experience working with children and families. A valid Florida driver’s license and auto liability insurance are required. Must have knowledge of mental health disorders. Ability to document all services in an electronic health record is necessary.
