Crisis Stabilization Unit (CSU) Aftercare Specialist

The Aftercare Specialist coordinates and partners with the individual, the family’s natural supports to assist the individual/family with the natural adjustment that comes following reintegration to the community. This position does require traveling and the use of personal vehicle with occasional use of a company vehicle. Individual must be well organized, able to multitask, creative, manage caseload with families and workers along with completing required paperwork, and good time management skills. The Aftercare Specialist will be responsible for completing exit interviews and brief follow up case management focusing on ensuring the individual links to the resources they were referred to; accessing additional and community connections for any individual. This is a 100% direct service position that could require availability to work irregular hours, including evenings, overnights, and weekends. Education: Minimum of a Bachelor’s Degree in Behavioral Sciences, Human Services or related field. Experience: Preferably a minimum of (1) year experience heavily concentrated in the mental health field. Work experience with adults with severe and persistent mental illnesses or with individuals with similar human-services needs as an agency case manager.
