Centralized Receiving System (CRS) Benefits Acquisition Specialist

The Benefits Acquisition Specialist performs coordinated screening and assessment of the benefit needs of the individual and is a source of support. This service is focused on utilizing the SSI/SSDI Outreach, Access, and Recovery (SOAR) Model which dramatically expedites the SSI/SSDI application process and reduces the disability determination period. The Specialist develops a supportive relationship with individuals served in order to assist with ongoing, systematic support to the person served including: completing necessary SOAR application paperwork and supporting documentation, facilitating of linkages with community resources, monitoring of overall application process and referrals of the individual; advocating for the individual with the Social Security Administration (SSA) and Disability Determination Services (DDS) Offices as well as external sources. Services are provided in accordance with accepted standards of professional practice and policies and procedures of HBH while assuring that the unit operates within the guidelines established by the Agency for Health Care Administration, CARF, and Department of Children and Families in a manner that is consistent with HBH mission, vision and values. This is a 100% direct service position that could require availability to work irregular hours, including evenings, overnights, and weekends. Minimum of a bachelor’s degree in behavioral sciences, Human Services or related field. Master’s degree in psychology, social work, counseling education, mental health counseling, or related field; preferred. Certifications obtained within 90 days after hire: AHCA approved Mental Health Targeted Care Coordinator Training, Crisis Prevention Intervention (CPI). Certification in Care Coordinator preferred. Preferably a minimum of (1) year experience heavily concentrated in the mental health field. Work experience with adults with severe and persistent mental illnesses or with individuals with similar human-services needs as an agency case manager desired. Additionally, experience in the mental health and/or substance abuse arena is preferred. Previous experience in discharge planning, utilization review, Case Management, or similar role desirable. Experience with adults with human-services needs. Familiarity with the various resources within the local community. Awareness of the concept of individual service planning. Demonstrated knowledge of 10E-15 Florida Administrative Code. Knowledge of both medical complexities including HIV/AIDS, Dementia, Alzheimer’s, Diabetes, and physical disabilities and special postulations (women, seniors, veterans, and those from outside of the USA) and how it might impact a person’s overall functioning and their treatment plan.
