Case Manager – Housing and Health Options Promote Empowerment (HHOPE) Program

The position requires a compassionate and dedicated individual to provide targeted case management services to adults with serious mental illness and individuals experiencing homelessness. The Case Manager assists persons served with developing and achieving individualized goals, accessing housing and community resources, obtaining benefits and mainstream services, and coordinating behavioral health and supportive services that promote recovery, stability, and independence. Qualified candidates must possess a bachelor’s degree, master’s preferred in a human services-related field and have at least one year of experience working with adults with behavioral health diagnoses; experience serving individuals experiencing homelessness is preferred. Candidates should demonstrate strong communication, advocacy, and interpersonal skills, knowledge of community resources and person-centered service planning, and familiarity with Medicaid-funded behavioral health services. A valid Florida driver’s license and proof of automobile insurance are required.
