Bachelor’s degree in behavioral health, social services, human services, healthcare, psychology, social work, or a related field preferred. This position is responsible for providing community-based care navigation and coordination services and must be able to travel throughout the community to meet the needs of individuals who are recent discharges from acute care settings and/or identified high utilizers of the behavioral health system and who may benefit from intensive, time-limited services focused on individuals transitioning from higher levels of care. This position is part of the organization’s broader continuum of care coordination. The position provides structured outreach, caring contacts, warm handoffs, service linkage and barrier resolution following acute level of care discharge. The Care Navigator’s primary responsibility is timely post-CSU engagement and navigation, to ongoing behavioral health, substance use, primary care, and community-based services; ensuring that the individual has a meaningful connection to ongoing care and does not become lost during the transition between systems by facilitating appropriate referrals and warm handoffs and helping ensure continuity while responsibility transitions from the acute care setting to the receiving care team. Accordingly, the Care Navigator’s primary focus is to support wellness, community integration, continuity of care, and successful transitions to ongoing services, while supporting organizational efforts to meet applicable HEDIS, payer, contractual, and quality requirements for timely post-discharge follow-up. The Care Navigator works collaboratively with discharge planners, case managers, peer specialists, primary care providers, community agencies, and other service providers to facilitate successful transitions and ensure that the individual remains connected to the appropriate level of care. Experience with computer technology (using computer & PC software to support activities; Windows based programs, e-mail, especially Microsoft Office applications, keyboarding skills, MFP) and other office tools efficiently to meet performance goals necessary to the position. This is a 100% direct service position.
