This position requires the candidate possess a minimum of a Bachelor’s degree; Master’s preferred. The position requires the candidate possess a minimum one year experience heavily concentrated in the mental health field; experience in discharge planning, utilization review, case management with adults with SPMI, or with individuals with similar human-services needs that are forensically involved. The candidate will be responsible for performing comprehensive discharge planning/case management functions obtaining linkage(s) for individuals with simple and complex needs and coordinates with community-based services with the primary focus on working with patients in crisis as defined by Chapter 394 of the Florida Mental Health Act law and are sent to the CSU (Crisis Stabilization Unit) via court ordered and forensically involved cases as patients at the CSU.
