Care Coordinator – Safety Management Action Response Team (SMART)

This position requires a bachelor’s in human services or related field; 1 year of relevant Social Services experience with children and families preferred. Specific agency training will be completed within the first six months of employment. Wraparound certified with experience working with families at risk preferred. Ideal candidate must possess excellent engagement, computer, time management skills and be able to work closely within a small team. Provide safety management services and care coordination for families with active involvement with DCF to ensure that appropriate interventions and services are provided to address caregiver protective capacities while ensuring the child’s safety, permanency, and well-being in relation to the identified needs of the family. Document activities and other services in FSFN. Possess a valid Florida Driver’s License. Provide proof of appropriate automobile insurance coverage in the amount of at least $10,000/20,000/10,000 for liability limits.
