This position requires a minimum of a bachelor’s degree in Behavioral Sciences, Human Services, or a related field. The Care Coordinator serves as a guide responsible for coordinating care for clients with chronic and/or high-risk behavioral health conditions, bridging gaps in services and supporting engagement in treatment and recovery. Responsibilities include completing FASAMS and treatment plan reviews, coordinating referrals, integrating behavioral and primary care, monitoring treatment adherence and access to services, and linking clients to community-based resources. The role also requires accurate and timely documentation in the electronic health record, completion of outcome measurements, collaboration with staff to improve care delivery, and meeting productivity standards. knowledge of co-occurring disorders and compliance with Medicaid, Florida regulations, and organizational policies required. This is a 100% direct service position. A valid Florida driver’s license and auto liability insurance are required.
