Adult Case Manager

Performs coordinated assessment of the needs of the person served. Provides case management functions including those of coordination of service planning, facilitation of linkages with community and center resources, monitoring of overall service delivery and referrals of the person served to sources providing, assistance with basic human needs, such as food and shelter. Advocates of the person served with center as well as external sources. Develops a supportive relationship with person served in order to assist with education and empowerment of the individual. Provides ongoing, goal oriented and systematic support to the person served. Bachelors or master’s degree in Human Services field. Some housing experience preferred (familiarity with assisted housing process and available resources). At least one-year experience with adults, preferable with mental health problems. Demonstrate good writing and organizational skills, community resource awareness, strong interpersonal skills, culturally sensitive, experience working with adults with mental health problems. A valid Florida driver’s license and auto liability insurance are required.
