Bachelor’s degree or master’s degree in a human services field from an accredited college or university. Minimum of one (1) year of experience working with children, youth, and families for general Targeted Case Management functions and two (2) years of relevant experience for Wraparound Facilitation responsibilities. The Youth Targeted Case Management (TCM) / Wraparound Facilitator Team Leader provides leadership, coordination, direct service support, and day-to-day oversight for a community-based program serving children, youth, and families with behavioral health and complex service needs. The Team Leader will support program start-up and implementation, staff development, referral coordination, quality assurance, documentation, community partnerships, and effective service delivery. The Team Leader will provide guidance, coaching, and case consultation to Targeted Case Managers and Wraparound Facilitators while ensuring services are family-driven, youth-guided, strengths-based, culturally responsive, trauma-informed, and coordinated across systems. The position requires collaboration with youth, families, providers, schools, child welfare, the Department of Juvenile Justice (DJJ), community organizations, and other supports. The Team Leader may maintain a reduced direct-service caseload as operational needs require. Possess and maintain a valid Florida Driver’s License and automobile insurance coverage with liability limits of at least $10,000/$20,000/$10,000. Complete required Wraparound training within three (3) months of hire.
