This position requires Bachelor’s degree in Sociology, Psychology, Social Work or related field. Master’s Degree preferred. Two years of related experience with case management services working with persons with SMI and or concurrent substance use disorders. One year+ experience working with HUD, locating housing, negotiating leases and providing in-home services. The Case Management Team Leader is specialized in working with individuals receiving housing vouchers. Their role includes traditional case management and additionally an understanding of housing for people with mental health conditions, and funding from HUD (The US Department of Housing and Urban Development). This person will have extensive experience working with both HUD housing and with individuals diagnosed with a serious mental illness who may also have a concurrent substance use disorder. Work involves orienting all eligible participants to the program and providing housing search, support to maintain housing and any other treatment and supportive services needed to promote participants self-sufficiency, integration into the community, and permanency in housing; performing administrative tasks involved in the review and maintenance of a caseload of program participants. The principal duties are performed both in a general office environment and in the field and community where program participants reside. Valid driver’s license and auto insurance required.
