Medicated Assisted Treatment (MAT) Program Care Coordinator

This position requires a Bachelor’s Degree. Candidate should have experience working with individuals with mental illness and substance use. Minimum of one (1) year case management experience is required. This service provider serves as a guide who is responsible for overseeing the community-level service plan in an effort to bridge the gaps in an individual’s care whose interventions and treatment are focused on helping individuals navigate the road to recovery as they move through the healthcare system. Services covered under care coordination include: Outreach, Assessment, Crisis Support/Emergency, Recovery Support, Case Management, Intensive Case Management, In-home & On-site, Supportive Housing and Intervention. This is a 100% direct service position that could require availability to work irregular hours, including evenings, and weekends.
