This service provider serves as a guide who is responsible for overseeing the community-level service plan in an effort to bridge the gaps in an individual’s care whose interventions and treatment are focused on helping individuals navigate the road to recovery as they move through the healthcare system. Minimum of a Bachelor’s Degree in Behavioral Sciences, Human Services or related field. Master’s degree in psychology, social work, counseling education, mental health counseling, or related field; preferred. Certification in Care Coordinator preferred. Preferably a minimum of (2) years experience heavily concentrated in either the mental health or substance use fields. Experience working with individuals with mental illness and substance use preferred. Minimum of 1-year Case Management Experience. Knowledge of Medication Assisted Treatment protocol desired.
