Coordinates the development and implementation of an individualized, strength based, youth driven, plan of care for all transitional youth residing at Wilson Gardens apartments. Provides case management functions including those of coordination of service planning, facilitation of linkages with community resources and monitoring of overall service delivery. Also provides independent living skills assessment and training, role modeling and coaching to achieve required competencies. Advocates for the person served, in the community. Develops a supportive relationship with the person served in order to assist with educational, employment and daily living needs. Possess a valid Florida Driver’s License. Provide proof of appropriate automobile insurance coverage in the amount of at least $10,00 20,000/10,000 for liability limits.
