Coordinates the development and implementation of an individualized, strength based, youth driven, plan of care for all transitional youth residing at Wilson Gardens apartments. Provides case management functions including those of coordination of service planning, facilitation of linkages with community resources and monitoring of overall service delivery. Also provides independent living skills assessment and training, role modeling and coaching to achieve required competencies. Advocates for the person served in the community. Develops a supportive relationship with the person served in order to assist with educational, employment and daily living needs. Candidate is required to have a bachelor’s degree in the human services field, and have at least one (1) year experience working with adolescents. Candidate must also demonstrate good writing and organization skills, community resource awareness, strong interpersonal skills, be culturally sensitive, and have experience working with youth and their families experiencing mental health issues. Candidate must also possess a valid Florida driver’s license, and be able to provide proof of automobile insurance coverage in the amount of at least $50,000/$100,000/$50,000 for liability limits.
