Functions as a core member of a collaborative care team that involves the patient’s primary care provides, a consulting psychiatrist and other primary/behavioral health team members. Performs coordinated screening and assessment of the needs of the person served. Provides care management functions including those of coordination of treatment planning, facilitation of linkages with community and Center resources, monitoring of overall service delivery and referrals of the person served. Candidate requires to have a degree in a Human Services field, and at least two years experience working with adults in a behavioral health and/or primary care setting. A valid Florida driver’s license and auto liability insurance are required.
