This service provider serves as a guide who is responsible for coordinating care for clients with chronic and or high risk medical and or behavioral health conditions. This provider bridges the gaps in an individual’s care. Interventions are focused on helping individuals engage in care and navigate the road to recovery. Reviewing records and data to ensure clients are accessing care in a timely manner and are attending appointments for recommended services, are following up with specialty care providers, are taking medications as prescribed and are accomplishing goals and objectives identified in treatment and or service plans. Care Coordinator will focus on the medically underserved population and health outcomes of clients. This Care Coordinator is located in our primary care location (HILL). Bachelors in Behavioral Health Services, Human Services or related field. Masters preferred.
