Minimum of a bachelor’s degree in Behavioral Sciences, Human Services, or related field. Master’s Degree in psychology, social work, counseling education, mental health counseling, or related field; preferred. This service provider serves as a guide who is responsible for coordinating care for clients with chronic and or high risk medical and/or behavioral health conditions. This provider bridges the gaps in an individual’s care. Interventions are focused on helping individuals engage in care and navigate the road to recovery. Services are provided in accordance with accepted standards of professional practice and policies and procedures of HBH while assuring that the unit operates within the guidelines established by the Agency for Health Care Administration, CARF, and Department of Children and Families in a manner that is consistent with HBH mission, vision and values. This is a 100% direct service position. A valid Florida driver’s license and auto liability insurance are required.
