As a member of the in-patient multidisciplinary team, the Licensed Practical Nurse, in conjunction with the Prescribers and the RNs, administers treatment and medications to person served, refers for physical exams and other physical problems, records medication orders, writes progress notes, makes telephone calls, reports to doctor regarding physical problems, assesses person served, ensures compliance with the Nurse Practice Act. Provide symptom education to enable individuals to identify their mental illness symptoms and medication teaching daily basis. Administer prescribed medications and treatments, including injections, and documents in medical record: demonstrate correct medication pass techniques, has checked appropriate vital signs before medication administration; chart action/reaction/treatments according to standards. Screens new clients (both in the screening and on the unit); assists with data collection, documentation in progress notes in accordance with nursing standards. Conduct intakes and orient clients. Assists/provides client psycho education, medication education, and health wellness education. Answer client questions or refer to qualified staff. Participate in education groups and document appropriately. Run assigned groups, such as medication groups. Monitor, evaluate and document individuals’ mental and physical conditions: documents appropriate data, interventions, notified doctor, and charge nurse when applicable. Assure safety of residents and security of the facility.
