The Children’s Care Coordinator works as part of our Children’s Care Coordination Team, with children transitioning from an acute inpatient level of care to community-based care, until they are effectively connected with services and supports. This includes but is not limited to services such as behavioral health, primary care, peer and natural supports, housing, education, and employment. Minimum of a Master’s degree in Behavioral Sciences, Human Services or related field. Minimum of (1) one-year experience working with children and families with complex needs. High Fidelity Wraparound desired. Additional related education and/or experience preferred. Bilingual (English/Spanish/Creole) a plus, but not required. A valid Florida driver’s license and auto liability insurance are required.
