This position is a Case Manager who is also designated to serve as a bridge between HBH and the community hospitals. This service is focused on a collaborative process of engagement, assessment/screening, planning, facilitation and advocacy for options and services to meet an individual’s behavioral health needs through communication and available resources. Minimum of a Bachelor’s Degree in Behavioral Sciences, Human Services or related field. Masters Degree in psychology, social work, counseling education, mental health counseling, or related field; preferred. A minimum of one (1) year experience heavily concentrated in the behavioral health field. Work experience with adults with severe and persistent mental illnesses or with individuals with similar human-services needs as an agency case manager desired. Additionally, experience in the mental health and/or substance abuse arena is preferred. Previous experience in discharge planning, utilization review, case management, or similar role desirable. Valid driver’s license and auto insurance of 50,000/100,000/50,000 required.
