This position requires a minimum of a Bachelor’s Degree in Human Services field, with 5 years of case management experience. Masters preferred. Other related degrees considered if supplemented by relevant work experience. Case Management experience highly desired, but other social work experience may suffice. Supervisory experience preferred. This position monitors the provision of case management functions by case managers/care coordinators including those of coordination of service planning, facilitation of linkages with community and Henderson resources, monitoring of overall service delivery and advocates for the participant within Henderson as well as external sources. Monitors performance measures and aids in data collection and reporting. Assists in establishing performance standards and monitors compliance. Participates as an active member of the center’s multidisciplinary staff. This position is responsible for the supervision of all case managers/care coordinators and navigator(s), and if applicable, requires direct service contact. This is a 20% direct service position that could require availability to work irregular hours, including holidays, evenings and weekends.
