Case Management Coordinator

This position requires a Bachelors Degree in a Human Service field with (5) five years experience; Master’s preferred. The candidate must have experience working with individuals having a behavioral health diagnosis. A working knowledge of available community resources, as well as an understanding of the HUD Homeless criteria. Familiarity with Evidence Based Practices, including Housing First, Trauma Informed Care and Motivation Interviewing. Candidate should have the ability to demonstrate success using EBP models preferred. Strong knowledge of housing models designed for the clients with behavioral healthcare needs and/or those with a history of homelessness required.
