Serves as a guide who is responsible for coordinating care for clients with chronic and or high risk medical and/or behavioral health conditions. Bridges the gaps in an individual’s care for both medical and behavioral health. Interventions are focused on helping individuals engage in care and navigate the road to recovery. Manage coordinate referrals, by integrating behavioral health and primary care (i.e. making appointments, getting reports back). Track and support individuals as they obtain services throughout the organization or within the community. In person or telehealth up with patients within 24 hours of an emergency room visit or hospital discharge. Perform comprehensive care management functions with the primary focus on linkage and engagement for individuals to community-based services either within the organization or outside per client’s choice. This position is located in our primary care department. A Bachelors in Behavioral Health Services, Human Services or related field. Masters preferred. Valid Drivers license and auto insurance required.
