This professional position oversees the entry and exit points of contact for the Acute Care Services BA/MA receiving & SRT facility. This individual is responsible for managing all new referrals and coordinating both the intake processes, as well as the discharge processes. The incumbent facilitates all phases of intake and discharge to include benefits verification, authorization and coordinates the intake process for inter-facility transfers, walk-ins, direct admits, and various referral sources. Monitors all facets of customer service, workflow, relationship management, and quality assurance in the screening and discharge departments. Services are provided with a Recovery Oriented System of Care approach and are delivered through intensive, time-limited, goal specific interventions. This position establishes/enforces standards of care; monitors to ensure it meets the organization’s standards, and reviews programmatic outcomes to analyze the effectiveness and efficiency of the care provided by the department. Communicates and provides consultation/training to staff and community agencies and partners. This individual provides administrative support to all staff. Lastly, the incumbent provides back up coverage, as necessary to ensure staffing appropriateness. This position reports directly to the Director of Acute Care Services. This position is responsible for the screening clinicians, discharge planners, aftercare specialists, students and volunteers, and auxiliary staff from the various contracts. This position has frequent contacts with prescribers, case managers, screeners, clinicians, discharge planners, other agency direct service providers, law enforcement, family members/significant others, area hospitals and staff, and other agencies, and maintains frequent contact with all other levels of management and staff, as needed. This staff develops and maintains professional relationships with program participants, all levels of HBH staff, managed care companies, families of consumers, and other professionals or community members who have an impact on those who we serve. This is a 25% direct service position that requires the employee to be accessible to deliver after hour on-call coverage by phone and face-to-face contact to staff and work and/or be available to work both non-traditional and irregular hours, including evenings, overnights, weekends and holidays.
